SAI Form

The parents should read about our SAI plan and then click on the link to take them to the forms page
The next page has our insurance forms defined by policy year dates. The very top set of forms is for this current policy year.
It is the parents responsibility to download and follow the instructions on completing the SAI claim form.
It is the Region’s responsibility to direct parents to the website and to be available to sign the forms and answer questions.
The parents are to be given the originals back along with a copy of the Incident Report form and the Region should keep copies of both.
A copy of the Incident Report form is mailed to the NSTC, Attn: Risk Management (address is on the form)
